Reviews: Troubleshooting guide

This guide covers errors you may see when setting up review types or when starting, completing or signing a review. For how review statuses, signatures and documents work, see Review signatures and documents overview and Review Statuses and Status logic. For setting up review types, see Review configuration overview.

Setting up reviews

Why do I see "Title must be unique", or an error when starting a review with custom questions?

Likely cause: two custom questions in the review type have the same title. Every question title must be unique within a review section, including sub-questions inside a Boolean Case Block question, because of how review data is stored.

You may see "Title must be unique" when you save the review configuration, or an error when someone tries to start the review.

What to do:

  1. In Programme Builder, open the programme and go to the reviews configuration.

  2. Find the custom questions for the review type showing the error.

  3. Rename any duplicates so each title is unique. If you need the same wording in more than one place, add a prefix, for example the name of the parent question.

  4. Save, then try again.

Why do I see "This learning activity does not have a linked review"?

What it means: the review type the learner's review component was linked to has been deleted from the programme in Programme Builder. The component stays in the learner's learning plan, but its review type no longer exists.

What to do:

  1. Edit the review component in the learner's learning plan.

  2. Choose a Review type that is still available on the programme.

  3. Complete any required fields and save.

To fix every learner on the programme, see the next entry.

Why do I see an error when starting a review, or saving a review section, after a review type was removed?

Likely cause: a review type was removed from the programme in Programme Builder after learners were enrolled. Adding the same review type back doesn't fix it, because the link between the review component and the review type is lost. This also stops reviews appearing in a generated Commitment Statement.

What to do:

  1. In Programme Builder, check every review component on the programme refers to a review type that still exists, then save the programme.

  2. For one learner on the programme, check each review component in their learning plan refers to the correct review type. Select the type explicitly, as the dropdown defaults to the first in the list.

  3. Synchronise the change to the remaining learners with Learning plan synchronisation.

To avoid this, don't remove a review type from a programme that has enrolled learners. If you need a new version, create a new review type with a version number, for example "Tripartite Review v1.2".

Using reviews

Why do I see an error when completing a custom review section with a ULN field?

Likely cause: the field collecting the ULN is set to the 'Numeric' field type. A 'Numeric' field holds a maximum value of 2,147,483,647, and a ten-digit ULN can exceed it.

What to do: in Programme Builder, change the field type for the ULN question to 'Text', then complete the section again.

Why do I see a blank error when saving a document or review?

Likely cause: one of two things.

  • Special characters typed together in a text field, especially /*, cause an internal error.

  • Hidden formatting copied into the Instructions field of a learning plan component, from a web page, Word document or email, carries into the review when the Learning Plan section is enabled, and the PDF fails to generate.

What to do:

For special characters, add a space between them, for example / *, and save again.

For hidden formatting:

  1. Open the learning plan component whose Instructions text was pasted from another source.

  2. Copy the text from the Instructions field into a Word document.

  3. Clear the Instructions field.

  4. Paste the text back from the Word document and save the component.

  5. Return to the review and complete it.

For learners already affected, fix one learner first, then use Learning plan synchronisation to update the others. Update the component in the programme build too, so new learners aren't affected.

Blank error message shown when saving a document or review

Why do I see an error when signing a review?

Likely cause: the review type has the Learning Plan and Learning Progress sections enabled, but the learner has no learning plan component in an active learning plan.

What to do: make sure the learner has at least one component in an active learning plan, or disable those sections for the review type in Programme Builder.
