Examples of printable Commitment & Agreement documents

Overview

The Aptem system provides many facilities to reduce the paperwork involved in gaining funding for apprenticeships, trainees. However, there are occasions when there is a requirement for a document to be printed for official recording within an organisation and for the individuals concerned. This article provides demonstration images of four important documents:

The Apprenticeship Agreement - basic apprenticeship agreement be in place at the start of the apprenticeship.

The Written Agreement - written agreement between the Employer and the main Training Provider.

The Commitment Statement - between the apprentice, employer, and training provider.

The UVAC Commitment Statement - between the apprentice, employer, and training provider.

Apprenticeship Agreement

An apprenticeship agreement must be in place at the start of the apprenticeship.
The purpose of the apprenticeship agreement is to identify:

the apprenticeship standard connected to the apprenticeship;
the dates during which the apprenticeship is expected to take place; and
the amount of off the job training that the apprentice is to receive.

The Written Agreement

This is a written agreement between the Employer and the Main Training Provider (Aven) and contains
the details of the End Point Assessment Organisation and any Subcontractors that may be
required to compliment the delivery of the Apprenticeship for the Apprentice detailed below.
This document lays out the planned delivery requirements of the Apprenticeship and the areas of
responsibility for each party with the relevant costs.

The Commitment Statement

This document outlines how all three parties (the apprentice, employer, and training provider) will support
the achievement of the apprenticeship. The document outlines the key milestones of the apprenticeship
and the learning plan designed to achieve this. The document also outlines information, policies,
procedures and guidance to support the achievement of the apprenticeship. The document is applicable
for Funding Year 19/20 onwards.

UVAC Commitment Statement

This Commitment Statement summarises the schedule, roles and responsibilities and funding that support the successful completion of this apprenticeship. This document also provides
evidence of the eligibility of this apprenticeship for funding set out in the Apprenticeship Funding Rules which govern the delivery of each apprenticeship.

● The content of this document forms the basis of the regular reviews between all three signatory parties. The reviews, set out in section 1.4, will track progress against the
knowledge, skills and behaviours gained during the apprenticeship, the recorded evidence of off the job learning by the apprentice, and agree the ongoing support and
commitment required from by all three signatories

● Any material changes to the apprenticeship, particularly dates or employer contacts, must be agreed and recorded in an updated Commitment Statement. Employer and
apprentice must hold an up to date signed copy of this document at all times. This will be distributed by the university to the parties listed in section 1.1 after any material changes
are agreed.
● Employers must notify the University as soon as possible if any changes in line manager or mentor occur, or there are any other changes that affect other elements detailed in
this document
● The main training components of this apprenticeship are contained at Section A. This Commitment Statement should be stored with the separate Apprenticeship Agreement
document.
