The most likely cause for this is that the learner has edited the referred piece of evidence which sets the status back to ‘Requires Marking’.

Please check the evidence history by first clicking the 'Mark Evidence' button on the learning plan component.



Click on the pencil by the evidence file.



Then click 'Show History'.

The history will be displayed.
